Job Description & Requirements

Registered Nurse – Case Management- Case Manager

This position is for an Offsite Case Manager. All work will be done at Non-Kaiser Hospitals, primarily in Pleasanton. Will primarily be based at Stanford/Tri-Valley 3-4 days a week. Gas mileage will be reimbursed for driving between the potential of 3 hospitals.

Required Qualifications

2 years of Case Management, Acute Care Experience Interqual MUST HAVE: InterQual (Admission Class, continued stay & discharge screen). 2 + years in direct patient care delivery. Exp in utilization review/Case mgt, and discharge planning, care coordination across the continuum of care in collaboration with the physician and other members of the health care team. Because we are also the Health Plan we do not have the requirement to get authorizations for hospitalization, but we do care about length of stay and appropriateness of admission and continued stay. Use Interqual on-line and MIDAS for avoidable days. Discharge Planning, explanation of benefits, Kaiser Health Connect, Care Port.
