Job Description & Requirements

Registered Nurse – Case Management- Case Manager

The Case Manager RN is responsible to monitor, coordinate, and facilitate the clinical management for an assigned patient population from admission to discharge to optimize clinical care, ensure effective and efficient use of resources, and enhance patient and provider satisfaction.

Required Qualifications

Reviews daily admissions to screen for discharge planning needs. Utilization Management Collects key insurance and clinical data on all patients BLS certification Travel exp Onsite position
