1. MO or compact RN license (#, expiration)
2. BLS, ACLS
3. NIHSS - may be submitted without but must obtain prior to start
4. Telemetry experience
5. Must be fully COVID and flu vaccinated
6. SSN/DOB and current reference required to send
7. ALL CONTINGENCY REQUESTS MUST BE ON THE SUBMISSION FORM
