Registered Nurse – Case Management- Case Manager

You'll work with adult patients of all ages, pediatric patients, and psych patients for case management. The hospital is in an active, desirable urban area. A vaccine declination is accepted for those with religious or medical exemptions. In this nurse case manager position, you’ll execute the hospital’s discharge planning or utilization review, or a dual role for both. You’ll implement a long-term care plan for patients in collaboration with the hospital’s medical team.
