Registered Nurse – Home Health
The Registered Nurse will be responsible for the prescription of nursing interventions, the coordination of patient care and activities, and the delegation of those aspects of care which may be provided by others. Supervises LPN's, HHA, as needed. Perform other activities and/or projects as directed by the Clinical Manager. The nurse will perform all duties in accordance with the mission, vision, and values of Mercy Health Center.
The Registered Nurse supervises the initiation, implementation and the delivery of care in the home health setting as well as monitors the effectiveness of services provided, assures that the appropriate home care services are provided and documented based on the specific patient need and in accordance with agency policy, provides comprehensive care coordination with the interdisciplinary team to ensure effective exchange of information related to the patient care, provides skilled nursing assessment, intervention and evaluation of the home care patient, document assessment of the patient’s condition and status per department policy, demonstrates the ability to utilize effective communication skills, require minimal supervision and is self-directed, implements and coordinates nursing care to assist the assigned primary patients and family to achieve their mutually identified individual goals, promotes self-care and independence, demonstrates professional conduct and evaluates her/his nursing performance based on accepted professional standards of practice, completes competency checklist during orientation and at least annually, meets needs of customers, department and organization by demonstrating flexibility in schedule, based on the department needs.
